The original open date/freeze date of the ham was Dec. 9, 2015. The ham was date marked one day past seven days. Corrected on-site, put the correct discard date. (Note: Educated to include open/freeze date as day one, then when thawing the product to count as day two and so on.)

KDA_3_50118A2 In the Beverage Air reach-in cooler, there was an open bag of shredded lettuce without a date. The person in charge said it was opened yesterday (past 24 hours). Corrected on-site, date marked the bag.

The head cook said they were placed out 10 minutes after 8 a.m. and the temperatures were taken around 8:30 a.m. (less than four hours). Corrected on-site, placed them in a reach-in cooler.

The person in charge said they have been out for about 15-30 minutes. Corrected on-site, placed them in the steamer to cook.

KDA_3_50116A1 At the steam table, there were two metal food-grade pans of nacho cheese sauce at 82.1 F and 80.6 F (middle temperature). Around the edges the temperature was reading 165 F. An employee said she placed the nacho cheese sauce on the steam table around 8 a.m. and the temperature was taken around 10:15 a.m. (less than four hours). Corrected on-site, placed the nacho cheese sauce in the steamer to reheat to 165 F. Now the nacho cheese sauce is reading 165 F-plus.

Corrected on-site, kept the unit closed until lunch time.

KDA_3_30212 In an upper cabinet, there was a plastic food-grade squirt bottle with clear liquid in it and it was labeled dark vanilla. The person in charge said it was white vanilla. Corrected on-site, labeled the bottle correctly.

KDA_7_10211 In the food-prep sink, there was a white plastic bucket with clear liquid in it and it was not labeled. An employee said it was bleach water. Corrected on-site, placed the sanitizer in a bucket that was already labeled “sanitizer.”

KDA_3_10111 In the dry food storage room, there was a crate that had two rotting red onions. There was white mold on one and they were both squishy. Corrected on-site, discarded.

An employee was wiping down the salad bar equipment. The manufacturers label is 200 ppm. Corrected on-site, replenished the bucket with room temperature water and Quaternary Ammonia and it measured at 200 ppm.
