KDA_4_90311A There was one box of gloves being stored directly on the floor in the basement. Corrected on-site, removed off of the floor.

KDA_6_20215A3 The middle, south side front door has a one-half inch gap along the bottom. This could allow the entrance for pests.

KDA_6_30111 There was no soap available at the downstairs hand sink. Corrected on-site, the person in charge replaced the soap in the dispenser.

KDA_4_70211 The mechanical dish machine located at the south bar was in use. It utilizes chlorine for sanitization and the concentration was at zero parts per million. The chlorine container was empty. Corrected on-site, replaced the container and primed the machine. Now it is reading 75 ppm and rewashed/resanitized the dishes.

KDA_5_20515B The floor drain below the ice machine located in the basement is slow to drain. The unit was being serviced during the inspection.

KDA_4_60111A On the rack above the three-compartment sink, there was one metal food-grade container with dried food debris inside and stacked clean. The meat slicer located downstairs had dried food debris on the blade and stored clean. On the utensil rack located in the basement, there was one metal food-grade pan with dried food debris on the outside and stored clean. Corrected on-site, cleaned/sanitized the meat slicer and moved the containers to the ware-wash area.

KDA_4_60211E4 At the north bar soda fountain machine, there is minor mold inside of the pop nozzles.

KDA_6_50112A There is grease and food debris on the floor below the grills.

KDA_3_50116A2 In salad make table, there was a metal container with hard-boiled eggs (shells off) at 47 F, 47 F and 48 F. There was a metal container of shredded parmesan cheese at 45 F. In the top drawer, there was a metal container of mozzarella sticks at 48 F. The kitchen manager said the lid and doors have been opened/closed due to lunch rush (11 a.m. to 1 p.m.). The temperatures were taken around 1:10 p.m. (less than four hours). The ambient air temperature of the unit was reading 35.2 F. Corrected on-site, discarded the eggs. Kept the lids closed. In the top right grill cooler drawer, there were raw ground beef patties at 45 F, 48 F and 48 F. The kitchen manager said the drawers have been opened/closed due to lunch rush (11 a.m. 1 p.m.). The temperatures were taken around 1:20 p.m. (less than four hours). The ambient air temperature of the unit was reading 40.3 F. Corrected on-site, placed the raw ground beef in the freezer to help cool down quickly. In the south side bar reach-in cooler, there was half-and-half at 49 F. The bartender said he has not used the product today but he has opened/closed the doors to serve drinks. The temperature was taken around 1:40 p.m. and the product has been in the cooler for more than four hours. The ambient air temperature of the unit was reading 40.8 F. Corrected on-site, discarded.

KDA_4_90311B On the rack above the three-compartment sink, there were two metal and two plastic food-grade containers stacked wet/clean and not able to properly air dry. At the north side bar, there were seven plastic cups stacked wet/clean and not able to properly air dry.

KDA_6_30112 There were no disposable paper towels available at the downstairs hand sink. Corrected on-site, the person in charge replaced the disposable paper towels in the dispenser.

KDA_6_50111 There are five broken floor tiles located in the women’s restroom.

KDA_3_50118A3 In the south side bar reach-in cooler, there was a container of Bloody Mary mix dated Nov. 16-23 (dated past seven days). In the north side bar reach-in cooler, there were two containers of Bloody Mary mix dated Nov. 19-16 (dated past seven days). Corrected on-site, put the correct discarded dates.

Corrected on-site, date-marked the cartons. There was also a bottle of Bloody Mary mix (made on-site) without a date. The person in charge said it was made on Nov. 12. Corrected on-site, discarded.

KDA_5_20515B The women’s restroom hand sink is slow to drain.

KDA_4_60211E4 Inside of the ice machine, there is mold on the plastic ice deflector. At the soda fountain machine, there is mold inside of the pop nozzles.

KDA_7_20111B There was a working spray bottle of glass cleaner being stored with the pepper shakers. Corrected on-site, removed the chemical. (Note: No leakage detected.)

KDA_3_30412E The metal ice scoop was stored on top of the ice machine (on the ledge). This is not a sanitary surface.

KDA_3_30211A1b In the walk-in cooler, there was a box of raw bacon being stored directly on top of a box of ready-to-eat hamburger buns. Corrected on-site, removed the box of bacon to a lower shelf. (Note: No leakage detected.)

4-102.11(B)(1) In the refrigerator, there was a plastic non-food grade “thank you” bag with limes in direct contact with the bag. Corrected on-site, placed the limes on a serving tray.
