2-301.14(G) There was an employee on the cook line in the back kitchen area that pulled out a raw egg with their bare hands, proceeded to crack the egg in a wok on the stove, then added raw shrimp along with a bowl of cooked rice. Employee grabbed a clean plate and placed the shrimp and rice dish onto the plate. Employee didn’t wash hands prior to touching the clean plate and plating the food. Corrected on site, educated the employee when to wash hands, employee washed hands and the shrimp and rice dish was discarded.
5-205.11(A) There was a wire mixer attachment along with a strainer full of rice in the basin of the hand sink located next to the walk-in cooler. When washing my hands, the sink began to fill up with water. Corrected on site, wire mixer was removed and the strainer was cleaned out.
6-301.14 There was no hand washing signage posted at the hand sinks located in the sushi station, or at the bar area. Corrected on site, hand washing signage was posted.
3-101.11 There was an orange being stored in a produce box with other oranges on a shelf in the walk-in cooler that had green mold growing on it. Corrected on site, item was discarded.
3-302.11(A)(2) There were two plastic open trays of raw steak being stored on a shelf directly over two open flats of raw shell eggs in the reach-in cooler. Corrected on site. Educated the person in charge about using the silo method for storing raw proteins and the raw eggs were placed above the raw steaks.
4-601.11(A) There was a commercial can opener mounted on a food prep table located across from the walk-in cooler that had a heavy amount of dried food debris on the blade. Corrected on site, person in charge placed in the dish room for cleaning.

4-602.11(E)(4) There is a black mold buildup on the inside of the ice machine deflector located in the back kitchen area. Corrected on site, person in charge cleaned with a sanitized towel.
7-102.11 There was a working spray bottle with a blue liquid hanging on the oven handle located on the cooks line that had no common name on the bottle. Person in charge stated it was degreaser. Corrected on site, chemical was marked.
7-201.11(B) There was a working spray bottle of degreaser hanging on the oven handle where the spray nozzle was pointing in the direction of the oven located on the cooks line. Corrected on site, chemical was removed.
3-501.15(A) There was a plastic lexan pan of cooked white rice with an internal temperature of 127.6 degrees. Person in charge stated it was just cooked about 20 minutes ago and placed on the shelf to cool. Corrected on site, person in charge had employee place in metal pans and put on ice to start the cooling process.
6-202.13(B)(1) There is a sticking fly trap used as a part of the establishment’s pest control that was being stored over a box of tempura batter located in the dry storage room.
6-501.111(A) There were four live, small flies and seven to 10 live house flies that were flying around in the kitchen area. Person in charge stated they have a pest control company. Paperwork was provided. Last service was on Aug. 8, 2014. Nothing was noted on the statement.
4-202.11(A)(2) There was a chef knife being stored as clean on a wall above the food prep table located across from the walk-in cooler that had a piece of metal chipped away from the top of the blade. Corrected on site, person in charge placed in managers office to have sharpened.
4-601.11(C) There is dried grease buildup on the hood filters located on the cook’s line. There is dried food buildup on the outside doors of all the reach-in coolers. There is dried food debris buildup on the inside shelfs of all the reach-in coolers.
5-202.13 There were two black hoses attached to a Y-splitter with shut off valves down from the mop sink faucet where one hose went to the chemical dispenser which has an air gap, and the other black hose with spray nozzle was being stored below the flood rim. Corrected on site, person in charge hung the black hose above the flood rim.
5-501.17 There is no cover on the trash receptacle located in the employee unisex restroom.
6-501.11 There are ceiling tiles in the server areas that are stained.
6-501.12(A) The outside red awning located above the entrance of the establishment has a heavy amount of pigeon fecal matter. Corrected on site, person in charge had employees power wash the awning. There is dried dirt and food buildup on the floors in the kitchen area.
